TAPESTRY (Teams Advancing Patient Experiences: Strengthening Quality) is a Health Canada funded project which will aim to help older adults in our practice stay healthy at home, functioning independently, for as long as possible.

McMaster Family Practice physicians Doug Oliver, Cathy Risdon and Gina Agarwal are all taking on significant leadership roles in the design of this study, which will see trained primary health care volunteers, personal health medical records and innovative tools used to help patients and their families navigate the healthcare system, all integrated into the network of care offered to our patients.
